MLA first page header belongs on the top left side (double-spaced) of the paper as follows:
Your name
Teacher's name
Course
Date due written out
Example:
Joe Brown
Ms. Costello
English 3 College Preparatory

Your subsequent pages should have your last name space page number on the top right side of each page as follows:
Brown 2
Your title should be centered, NOT bold, underlined, in quotations, italicized or in any other font or size.
It SHOULD have all important words capitalized as usual and be related to your topic/thesis.
For example:

Other general rules:
Paper is 12 point font, double spaced, with one inch margins.
